NDIS Engagement Officer - Sunbury Community Health

Employment type: Part Time (negotiable) Fixed Term Contract until December 2018

We are recruiting for a newly created NDIS position to join our skilled and supportive Child Development Team.

We are seeking an enthusiastic Grade 2 Occupational Therapist; Grade 2 Speech Pathologist; Early Childhood Teacher or a relevant Allied Health Clinician who has experience in working with children (0-12 years) with additional needs, their families and community service providers.

The position is specifically to engage with families as they enter our service, including provision of information regarding access to NDIS funding and service systems.

Knowledge and experience in family-centred and strengths based practice is essential.

Sunbury Community Health builds the social fabric of our communities through diverse services and programs that help people support themselves and each other.Our Health and Wellbeing Unit is seeking a Care Manager in our Home Care Package program. Applicants must have a tertiary qualification in Allied Health, Social Work or Nursing and have experience in goal directed care planning and coordination of services using the principles of consumer directed care.
